Benefit Description
This benefit may provide living accommodation at a Foresters-appointed facility for eligible members who are at least 65 years old and are indigent.
In addition to accommodation, Foresters may also provide each eligible member resident a monthly allowance and may, at its sole discretion, through a benevolence fund, provide an additional amount.
Eligibility requirements and limitations
To be considered for admission to a Foresters-appointed facility, an eligible member must satisfy the following requirements:
- The eligible member’s income meets his/her country of residence’s government guidelines for low income and assets are equal to or less than annual government guidelines. The country of residence must be the United States or Canada.
- The eligible member must qualify for Medicare or the Medicaid program in
his/her state of residence in the United States, or for medicare in his/her province of residence in Canada.
- The eligible member must hold an inforce social fraternal membership, Foresters life insurance certificate, or a non-Foresters life insurance certificate (Canadian residents only) issued between January 1, 1998 and October 31, 2000 and purchased in Canada through Foresters Financial.
- Be an eligible member on the date of application for this benefit and have been an eligible member continuously during the 15-year period immediately preceding the application date.
- The eligible member must have reached age 65 before the date of application for this benefit (if the member’s Foresters certificate of life insurance matures or expires within seven months of his/her 65th birthday, he/she may submit an application for admission to the Foresters-appointed facility within 90 days before this maturity or expiry date).
- The eligible member must complete a Foresters application form and consent to a thorough financial and health background check.
- The eligible member must be of sound mind, in good general health with no contagious and infectious diseases, and must be capable of self-maintenance. The eligible member must be willing to submit to an examination by a legally qualified physician selected by Foresters and a psychological evaluation provided at the Foresters-appointed facility with regard to his/her physical and mental capacity. The psychological evaluation or examination can result in denial of admittance to the Foresters-appointed facility if it is determined that the eligible member’s lifestyle would not be appropriate for communal living in a humanistic and
fraternal environment.
- The eligible member must sign a continuing care contract.
- On approval of his/her application, the eligible member will assign and transfer to Foresters all right, title and interest in all real and personal property (including motor vehicles) which he/she owns or is entitled to, and will undertake and agree to transfer and deliver to Foresters any and all of such property that may thereafter come into his/her possession.
- If an eligible member’s spouse is not a holder of his/her own social fraternal membership, Foresters life insurance certificate, or non-Foresters life insurance certificate issued between January 1, 1998 and October 31, 2000 and purchased in Canada through Foresters Financial, at Foresters discretion his/her spouse may reside with the eligible member at the Foresters-appointed facility and may receive maintenance support. In exercising such discretion, Foresters will take into consideration the age and health of the spouse, the number of years married to the eligible member and such other matters as it may consider necessary and proper.
- This benefit does not include skilled nursing care and will end if such care is required.
